Two litres a day, no added salt, and a persistent headache that I spent a month attributing to the medication.

The test is quick: add fluid and salt for two days and see whether the symptom goes. If it does not, it was not hydration and it is worth a panel.

What I actually want to know is why hydration goes wrong on this specifically, given that nobody is telling us to drink less.

Not looking for reassurance. Looking for the part I have got wrong.

Electrolyte monitoring on hydration — especially important during early titration when GI effects are most pronounced:

  • Sodium: 141 mEq/L (normal, but was low at week 2 from vomiting)
  • Potassium: 4.1 mEq/L (supplement if below 3.5)
  • Magnesium: 2.1 mg/dL (supplementing with glycinate)
  • Phosphorus: 3.5 mg/dL (normal)

Low electrolytes cause muscle cramps, heart palpitations, and fatigue — symptoms often blamed on the medication itself. Check your levels before assuming it's a drug side effect.

Muscle cramps on hydration — anyone else? I was getting severe leg cramps at night, especially calves. Bloodwork showed my potassium was low.

GLP-1 agonists can cause electrolyte shifts due to reduced food intake and GI effects. Get your electrolytes checked and supplement accordingly. I take magnesium glycinate 400mg + potassium gluconate 99mg daily and cramps are gone.

Can confirm. Same sequence, different timescale. I had assumed I was the exception until I read this.

From the other side of the consultation, briefly.

Helpful anti-nausea foods for hydration users:

  • Ginger (tea, chews, supplements — all work)
  • Peppermint (tea or oil capsules)
  • Plain crackers or rice cakes
  • Bananas (easy to digest, replenish potassium)
  • Clear broth (hydration + sodium)
  • Small amounts of protein (chicken breast, plain Greek yogurt)

Avoid: greasy foods, spicy foods, large meals, lying down after eating. Keep meals small and frequent — I do 5-6 mini meals/day.
