Nurse practitioner here. Practical tips for maximizing your vial's usable life: 1. Keep it refrigerated at all times except when drawing your dose. Take it out, draw your dose, put it back. Don't leave it on the counter. 2. Use proper aseptic technique every time you access the vial. Swab the stopper with 70% IPA, let it dry for 30 seconds, then insert the needle. This is the single biggest factor in preventing contamination. 3. Don't use the same needle to draw from the vial that you use to inject. Draw with one needle, swap to your injection needle. This prevents coring (punching a small piece of the rubber stopper into the vial). 4. Inspect before every use. Hold the vial up to a light and look for cloudiness, particles, or color change. If anything looks different from when you first received it, don't use it. 5. Write the date you first accessed the vial on the label. This is your "in-use" date. Some compounders assign a shorter BUD once the vial is first punctured (e.g., 28 days after first access regardless of the overall BUD). 6. Never re-enter a vial with a used needle. Seems obvious but you'd be surprised.

the point above about in-use dating is critical. There's a difference between: - BUD (Beyond-Use Date): From date of compounding, based on stability and sterility data in an intact container - In-Use Date: From date of first puncture, based on antimicrobial effectiveness and contamination risk Many compounders don't distinguish between these, but they should. Once you puncture that stopper, you've introduced a potential contamination pathway. Even with preservative and proper technique, the risk increases with each access. Best practice: if your vial has a 90-day BUD but you first puncture it on day 30, the in-use period (typically 28 days for multi-dose vials per USP <797>) would expire on day 58 — BEFORE the BUD. In that case, your effective expiry is day 58. This is why I always recommend patients get the smallest vial size that matches their dosing schedule within a 28-day window.

Mind blown. I had no idea about the in-use date being different from the BUD. So even though my vial says 60 days, if I understand correctly, once I puncture it I should ideally use it within 28 days? Going to ask my compounder about smaller vials. A 2mL vial at 5mg/mL would give me 10mg total, which at 0.5mg/week is 4 weeks — perfect for the 28-day in-use window. Thanks everyone.
