Got a question that I feel like nobody gives a straight answer to: how long is my compounded semaglutide actually good for? My vial says "beyond-use date: 60 days from compounding" but I've seen other compounders label theirs for 90 days, 120 days, or even 180 days. The branded Ozempic pen is good for 56 days at room temp after first use. Are these longer BUDs legit, or are some compounders just slapping extended dates on to make their product seem like a better value? For context, I have a 5mg/mL semaglutide vial, 5mL volume, from a 503B facility. I dose 0.5mg/week so this vial should theoretically last me 10 weeks (50 doses at 0.1mL each). But with a 60-day BUD, I'd need to toss it at week 8-9 with product still remaining.

Let me break down the regulatory framework: USP <797> (2023 revision) sets default BUDs for sterile preparations based on compounding conditions: | Category | Storage Temp | Default BUD | |---|---|---| | Category 1 (basic) | Controlled room temp | 12 hours | | Category 1 (basic) | Refrigerated | 24 hours | | Category 2 (more controlled) | Controlled room temp | 4 days | | Category 2 (more controlled) | Refrigerated | 10 days | To assign BUDs BEYOND these defaults, the pharmacy MUST conduct or reference: 1. Sterility testing (USP <71>) 2. Endotoxin testing (USP <85>) 3. Stability-indicating assay (potency over time) 4. Container closure integrity testing So when you see a 503B facility assigning a 90-day or 180-day BUD, they should have stability data supporting that claim. Legitimate 503B facilities will have conducted formal stability studies: potency testing at time points (T=0, 30, 60, 90 days etc.), sterility testing at each time point, and particulate testing. If a 503A pharmacy is assigning a 90-day BUD, I'd ask them point-blank: "Do you have stability data supporting this beyond-use date?" This is incredibly detailed, thank you. So the bottom line is: a 60-day BUD on a properly tested product stored refrigerated is totally reasonable and probably conservative? And the 180-day BUDs I've seen are... suspicious?

A 60-day BUD at 2-8°C for a well-formulated semaglutide solution is very reasonable and supported by typical stability data. 180 days is not impossible, but I'd want to see the data. Specifically: - Stability study with time points at 0, 30, 60, 90, 120, 150, and 180 days - Potency >90% at all time points - No significant increase in degradation products - Sterility maintained (no growth at all time points) - Endotoxin levels within limits - Particulate testing within USP <788> limits The challenge is that many compounders are assigning extended BUDs based on: - Published literature on the branded product (not directly applicable to their formulation) - API supplier stability data (for the raw material, not the finished preparation) - "Professional judgment" without supporting data All of these are problematic from a regulatory standpoint. USP <797> is clear: extended BUDs require preparation-specific stability data. My practical advice: use your vial within the labeled BUD, keep it refrigerated, and if you can't use it all within the BUD window, ask your compounder about smaller vial sizes or higher concentrations.
