It's a great point. Propylene glycol sensitivity is uncommon but real. If someone was having low-grade injection site reactions on Ozempic (which contains PG) and then switched to Wegovy (which doesn't), they might actually feel BETTER at the injection site despite the higher dose. Conversely, if someone has sensitivity to the higher NaCl concentration, Wegovy could sting more.

This is one of those areas where the "it's the same drug" oversimplification really fails patients. Biosimilars and reformulations are NOT always clinically interchangeable in terms of patient experience, even when bioequivalence is demonstrated at the population level.

I switched the other direction — Wegovy 1.7mg to Ozempic 1mg (supply issues, had to take what I could get). Massive difference. The Ozempic felt "weaker" which yeah, it IS a lower dose, but even accounting for that, the appetite suppression felt qualitatively different. Less "I'm not hungry" and more "food doesn't sound appealing." Hard to explain but it's a different flavor of appetite control.

^ this resonates hard. Ozempic appetite suppression for me = "I could eat but I don't need to." Wegovy appetite suppression = "the thought of eating makes me vaguely uncomfortable." Same molecule supposedly but very different subjective experience.

Lurker here. This thread is making me reconsider. My doctor offered me either and I just went with Ozempic because it was what I'd heard of. Maybe I should've thought about it more carefully. Is one generally considered "better" for weight loss?

Wegovy goes to a higher max dose (2.4mg vs 2.0mg for Ozempic), and it's the one with the formal FDA approval for chronic weight management. The STEP trials used the Wegovy formulation and titration schedule. So from a regulatory and clinical evidence standpoint, Wegovy is the one "designed for" weight loss.

Ozempic is FDA-approved for T2DM with cardiovascular risk reduction. Its use for weight loss is off-label (very common, but off-label).

That said, for many people Ozempic works perfectly well for weight loss, especially at the 1-2mg doses. If your insurance covers one but not the other, go with what's covered. The practical difference is small relative to the cost difference of paying out of pocket.
