Clinical perspective, offered as context rather than as advice.

Metabolic syndrome resolution on cardiovascular risk: I went from meeting 5 of 5 diagnostic criteria to meeting ZERO after 13 months of treatment.

The 5 criteria (and my journey):

  1. Waist circumference: 52" → 34" ✅ Resolved
  2. Triglycerides: 236 → 116 ✅ Resolved
  3. HDL: 36 → 58 ✅ Resolved
  4. Blood pressure: 146/94 → 119/73 ✅ Resolved
  5. Fasting glucose: 116 → 88 ✅ Resolved

Metabolic syndrome reversal is, in my view, the most medically significant outcome of GLP-1 therapy.

How much of the between-person variation is pharmacokinetic and how much is just adherence measured badly?

I will push back on the "any working dose is fine" framing. The maintenance evidence sits overwhelmingly at the top studied dose, and the extension data shows regain tracking dose reduction rather than tracking stopping. Holding low is reasonable; pretending it is evidentially equivalent is not.

Continuous metabolic monitoring dashboard for cardiovascular risk — I track everything in a spreadsheet and here's the month-over-month trend for my key markers:

Weight: consistent downtrend, -1.7 lbs/week average
Fasting glucose (finger stick): stable at 82 mg/dL
Blood pressure (home): 117/72 average
Resting heart rate: 62 bpm (down from 78)
Waist circumference: down 12 inches total

The resting heart rate improvement correlates with cardiovascular fitness gains. Everything is moving in the right direction.
