SUSTAIN-6 was the first CVOT to show cardiovascular benefit with semaglutide, relevant to cardiovascular risk.

NNT calculation for cardiovascular risk clinical endpoints: NNT = 1/ARR (absolute risk reduction).

From SELECT trial: MACE at 39 months — 6.5% semaglutide vs 8.0% placebo. ARR = 1.5%. NNT = 67 over 3.3 years.

Compare to established therapies:

InterventionNNTTimeframe
Semaglutide (MACE)673.3 years
Statins primary prevention (MI)~1005 years
Aspirin secondary prevention~772 years

These NNTs are clinically meaningful and comparable to accepted cardiovascular interventions.

This answered a question I did not know how to ask. Printing the relevant bit and taking it with me.

Metabolic syndrome resolution on cardiovascular risk: I went from meeting 4 of 5 diagnostic criteria to meeting ZERO after 9 months of treatment.

The 5 criteria (and my journey):

  1. Waist circumference: 48" → 36" ✅ Resolved
  2. Triglycerides: 238 → 103 ✅ Resolved
  3. HDL: 32 → 60 ✅ Resolved
  4. Blood pressure: 148/96 → 121/75 ✅ Resolved
  5. Fasting glucose: 118 → 84 ✅ Resolved

Metabolic syndrome reversal is, in my view, the most medically significant outcome of GLP-1 therapy.

Prescribed on cardiovascular grounds rather than for weight, and almost everything written for patients assumes the opposite.

Dizziness and cardiovascular risk: I was lightheaded for the first 3 weeks. Root cause was a combination of reduced caloric intake and mild dehydration.

Fix: minimum 80-100oz water daily, don't skip meals even if you're not hungry (eat small protein-rich snacks), and stand up slowly from sitting/lying positions. Also check your blood pressure — GLP-1-induced weight loss can make BP meds too strong, requiring dose reduction.

Moderator note: reminder that nothing in this thread is medical advice, and that clinical claims need a source.
